Oddball is correct this time. That order form is for the bass drum. Per the catalog #727 is for spurs attached to shell. #172 is for consolette holder and #590 is for shell mounted cymbal arm holder.

That makes a lot of sense actually. I'm definitely going to frame it or laminate it (Probably the former) because I want to preserve it with the kit. Definitely feels like a piece of history to me. I really want to contact the previous owner and ask him the history of the kit. Would love to have that written down to go along with it.It does make me wonder what the 172, 590, 727 numbers are.
Those numbers mean he bought it in Ma. The kit was 172 plus taxes and a fee.
