I believe it to be a model 1005 from around the late 70s (78-79).

Thank you for the info. I have done some more searching and I believe that it is a 10 lug Super Olympic model 1035 from 76 / 77. They only made these for 2 years, that is probably why I had such a hard time finding the info on it.
