New Throw-off for '58 Slingerland

I have a 1958 Slingerland Student Model Snare drum with the Rapid-Strainer throw-off. It doesnt work very well (often turns off while playing). I've resulted to tightening it so it's permanently on.

I would love to have it work.

My questions are:

Is this common?

Can I fix it?

If I can't fix it, is there a different Slingerland throw-off that I can buy that wont require any extra holes to install? or is there a Ludwig throw-off that can be installed without drilling?

The hole spacing is the same as the Ludwig p-85 and many replacement throw-offs, so you could replace the one you have without re-drilling.

I've had the same problem as you with other throw offs, but never with the rapid...
