I finally had a chance to look into this and it seems that a faulty insert might have been the culprit...I switched it out with a spare I had...So far, so good...

Seems to me that the threading in your lug casing that your t-rod screws into is deteriorating..You should take a look inside it to make sure.. It still screws on because it hasn't completely deteriorated, but more playing will definitely cause it waste away..

I've found that if the bearing edge is not perfectly flat, it causes this issue. A bass drum edge that has a higher point at one lug wouldn't need as much tension as the adjacent lugs. Therefore, if that rod has less tension on it, it is more likely to back out.
