When I find ones that have it missing, I actually drill a nickel. They work great.

I have had success with a thick fender washer. Even the hole is in the center (and not offset), it still works fine. Take the foot board with you to make sure you get the correct size. Any decent size hardware store should have "Thick Fender Washers" in a box of various sizes.
Just a heads up, if you need to replace the screw that holds the washer in (I think it's an 8-32 screw), I had a hard time finding a screw that was short enough (the shortest one I could find was "bottoming out" before it got tight). I solved this by using a few extra washers, including one lock washer. Everything works fine that way.
